Duties
Responsible for coordination of Start of Care, Re-certification, Discharge, and Resumption of careProvide direct patient care, including administering medications, performing diagnostic tests, and monitoring patient vital signs. Collaborate with physicians and other healthcare professionals to develop and implement patient care plans. Educate patients and their families about medical conditions, treatment options, and preventive care measures. Assist with discharge planning and coordinate post-discharge care for patients. Provide specialized care for patients with dementia or other cognitive impairments.
Qualifications
- Valid Registered Nurse (RN) Required in the state of Texas. 2. Minimum 2 years experience in home care experience 3. Effective written and verbal communication 4. Experience with policy, rules, and regulations 5. Clean background check and driving record Note: The specific qualifications may vary depending on the employer's requirements.
