Company Description Catholic Charities Family and Community Services supports individuals from diverse backgrounds as they work toward greater independence and stability. The organization provides housing, employment assistance, and coordinated care services for people experiencing challenges such as addiction, aging, mental illness, HIV, poverty, developmental disabilities, and immigration-related issues. Services are designed to promote dignity, respect, and self-sufficiency for each person served. Team members collaborate across programs to deliver holistic, community-based support that addresses both immediate needs and long-term goals. Role Description The Registered Nurse is a full-time, on-site role based in the Rochester, New York Metropolitan Area. This position provides direct nursing care, including health assessments, medication administration oversight, monitoring of chronic conditions, and coordination of care with physicians and other providers. The nurse educates individuals and families about health management, treatment plans, and preventive care, while advocating for client-centered, trauma-informed services. Day-to-day responsibilities include documenting clinical observations, maintaining accurate health records, participating in interdisciplinary team meetings, and responding to urgent health concerns as needed. The role also supports program initiatives by contributing to care planning, quality improvement efforts, and compliance with regulatory and agency standards. Qualifications Active Registered Nurse (RN) license in New York State and graduation from an accredited nursing program. Clinical skills in health assessment, medication administration, chronic disease management, and infection control. Experience working with populations affected by addiction, mental illness, aging, HIV, poverty, developmental disabilities, or immigration-related challenges is highly beneficial. Strong communication, documentation, and patient education skills, with the ability to build trust and rapport across diverse communities. Ability to collaborate within an interdisciplinary team, manage multiple priorities, and maintain professional boundaries. Knowledge of community health resources, care coordination practices, and relevant regulatory requirements. Proficiency with electronic health records and basic computer applications. Commitment to person-centered care, cultural humility, and the mission and values of Catholic Charities Family and Community Services.

Requirements

  • Active Registered Nurse (RN) license in New York State
