Position

Summary

Provides centralized follow-up services for PPNCNY. Responsible for reviewing and managing abnormal lab results, tracking referrals, and ensuring timely patient communication regarding next steps in care. Manages the nurse line and responds to patient inquiries consistent with clinical protocols and organizational standards. Collaborates with providers and health center staff to ensure continuity of care, accurate documentation in the electronic health record, and timely closure of labs and referrals. Supports quality improvement efforts related to follow-up processes.

Essential

Duties

  • Demonstrates ongoing proficiency in electronic practice management systems and the electronic health record (EHR).
  • Maintains working knowledge of health center policies, procedures, and medical protocols.
  • Reviews and manages abnormal lab results and referrals in accordance with PPNCNY Medical Protocols and Centralized Follow- Up policies; ensures timely communication and closure of care loops.
  • Manages the nurse line and MYChart, triages patient calls appropriately, provides education within scope, and consults or refers to a provider as needed.
  • Collaborates with providers and health center staff to ensure continuity of care and accurate, timely documentation.
  • Supports a culture of communication, teamwork, and shared problem-solving across sites and departments.
  • Participates in ongoing feedback and quality improvement efforts related to follow-up processes and patient experience.
  • Ensures all interactions reflect PPNCNY’s values of integrity, partnership, customer focus, communication, and quality.
  • Supports the agency’s mission and full scope of reproductive health services.
  • Promotes culturally and linguistically competent care and actively builds awareness of diversity and inclusion.
  • Complies with established protocols and regulatory requirements, including HIPAA, OSHA, CLIA, and other applicable standards.
  • Participates in agency committees, meetings, in-services, and staff development activities as directed.

Requirements

Required skills and abilities

  • Ability to organize, prioritize, and manage multiple tasks and data with accuracy, attention to detail, flexibility, and discretion.
  • Strong interpersonal skills and ability to work collaboratively with internal and external stakeholders.
  • Proficiency in Microsoft Office applications (Word, Excel) and electronic health record/database systems.
  • Commitment to PPNCNY’s core values, including teamwork, compassion, confidentiality, and quality care.
  • Understanding of and adherence to PPNCNY Personnel Policies.

Qualifications

  • Registered Nurse currently licensed to practice in New York State.
  • Minimum of one year of hospital or clinical nursing experience required.
  • Experience in women’s health, family planning, STI testing and treatment, or OB/GYN nursing preferred.
  • Demonstrated interest in patient counseling, education, and supportive care.
