About this role

We have an opening for an APP in our exciting geriatric mobile practice focusing on wellness and disease management in the geriatric community. Our team is supportive, caring, and we have tried to build a work environment that exemplifies teamwork.

Our focus is primary care in assisted living communities. We are offering a full time position to an APP to better serve our clients medical needs.

Being a mobile practice we go out to Assisted Living and Memory Care units to provide services. We go to 1 community each day and are in all of our facilities weekly. The current need is for facilities that are located in both Springfield and Decatur (Timberlake's Springfield & Randall Residence Decatur)

We have a full time LPN covering each community (1:1 ratio with NP) and a full time APP that is usually over 2-4 communities.

This position offers a lot of autonomy. Due to that autonomy and complexity of our patients, we do not hire new grads.

It is a very exciting practice that we have built over the past 10 years. We currently have successful practices throughout Indiana and Ohio and are opening new practices in Illinois, Kentucky and West Virginia this year. We are the preferred provider or medical director in each community we serve. Our reputation in the industry is vastly growing with many more communities ready to come on board this year. All we need is you!
