Requirements
- Must have an RN or BSN certification and possess the original document of certification from a state approved training provider.
- Must be able to provide original documents for CT Driver's License or CT State ID and original Social Security Card.
- Must be able to provide documentation of a physical and PPD taken within the past 12 months OR be willing to obtain a physical and PPD test quickly.

Duties
Registered Nurses (RN) Case Managers oversee all medical services our clients receive which includes nursing, physical / occupational therapy, and home health aides. This begins with a comprehensive clinical assessment and creation of a specialized care plan, consistent with recommendations from the hospital, skilled nursing facility, and primary care physician. Direct nursing services include periodic health assessments, teaching, skilled intervention, care coordination, and medication management. All of this takes place in the client's home!
Utopia provides a full spectrum of nursing services such as Medical / Surgical, Wound Care, Orthopedic Care, and Behavioral Health. This position requires patience, compassion, and a desire to care for those who need it most, just like family.
