About this role

Overview

The Home Health Aide (Medicare Certified) will provide home health patient/resident care and performs various treatments under the direction of the licensed nurse as required. The Home Health Aide (Medicare Certified) completes assigned tasks to ensure timely, efficient and safe patient/resident care. Communication and interactions with patients, residents, families and other health team members is an essential component of this position.

Responsibilities

Performs initial and ongoing data collection on home health patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse. Implements plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs. Provides direct patient/resident care and reports condition or status changes accurately and timely to the RN or LPN.

Demonstrates time management and prioritization of tasks. Documents results of patient/resident care within established policies, procedures and scope of practice. Follows facility policies on medication administration and treatments under the direction of a licensed nurse.

Qualifications required

Must be in good standing on the State Nursing Assistant registry or appropriate entity Valid Driver’s License Acceptable driving record that meets Benedictine requirements.

Qualifications preferred

Previous home health, hospital, or nursing home experience preferred BLS Certification.
