About this role
Home Health RN position at The Pines Home Health and Hospice. Primarily serves Prescott Valley to Cordes Lakes communities. The RN will build relationships with patients and families, create personalized care plans, coordinate care as a Case Manager, educate and empower patients and caregivers, and collaborate with an interdisciplinary team. Full Time: 4-5 days/week.
