Our organization is seeking qualified Registered Nurse (RN) - Case Manager performing visits with our geriatric population in their own homes. Responsibilities include comprehensive assessments ,individualized care planning, patient and family education, interdisciplinary care management, within the home setting. The nurse will work as a liaison between physicians, families and the administrative nursing team to ensure a continuum of care.
*The ideal candidate will have Home Care experience and the following
qualifications
- Willing to travel to multiple locations
- Medicare/Oasis Experience Preferred
- Valid RN License in good standing with the State of Michigan
- CPR/BLS Certification
- Ability to work independently as well as within a team environment
- Exceptional interpersonal & communication skills
- Outstanding organizational and self-management skills
- Proficient computer skills
- Auto Insurance and reliable transportation
- 1 year experience in Home Health Experience
Pay Rate shall be based on current per visit rates plus Mileage reimbursement
We are headquartered in Reston, VA with offices in Michigan, Virginia, Texas, and Louisiana. We provide a wide range of home health, hospice, private duty, catastrophic care, and personal care services.
