Family Tree Private Care is searching for a full-time RN Clinical Care Coordinator. The RN Clinical Care Coordinator position is full-time with competitive pay and benefits, flexible schedule options, an enjoyable work environment that is warm, positive, and supportive. Responsibilities include performing initial assessments in clients' place of residence, revising plans of care, serving as account executive for assigned clientele, collaborating with care coordination team, providing on-site training and education to Care Providers, supervising nursing personnel, obtaining physician orders, tracking quality metrics, using infection control measures, and performing on-call responsibilities.
Requirements
RN required
Minimum of 1 year of experience working in the home care industry (or related industry - hospice, home health, hospital, etc.)
Fluent in basic Microsoft Office functions (Word, Excel, PowerPoint)
Able to organize and maintain electronic client records
Strong interpersonal skills
Problem-solving and decision-making skills
Excellent written and oral communication skills
Able to manage time well and work efficiently
