Registered Nurse – Home Health at Elara Caring. Provides high-quality care to patients in their homes. Key responsibilities include aligning care with mission, maintaining HIPAA confidentiality, assessing patient and family needs, managing caseload, performing patient evaluations, communicating changes in patient condition, and coordinating care plans with interdisciplinary team.

Requirements

  • Current, unrestricted RN license
  • 1+ year clinical nursing experience
  • Willingness to travel within assigned territory (up to 50%)
  • Valid driver's license, reliable vehicle, and current auto insurance
  • Ability to lift/push/pull up to 50 lbs and assist with patient mobility
  • Associate's or Bachelor's in Nursing preferred
  • Home Health, Hospice, or HomeCare HomeBase experience preferred
