*Registered Nurse - Home Health*
*Responsibilities and Duties of the Registered Nurse*
- Perform full patient assessment including OASIS on admission, after hospitalization 60 re certification and upon patient change of condition of patients
- Participate in decision making process regarding patient care including patient clinical, functional and environmental and services needs.
- Create and modify the plan of care for patients
- Perform supervisory visits for LVNs and Aids
- Communicate with physicians, care coordinators and supervisors to present changes and needs of patents care
- Delegate LVNs to perform procedures, teach and Assess patient health care status.
- Write and supervise accurate home health aid plan
- Perform regular visits as needed
*Qualifications and Skills for Registered Nurse*
- An Registered Nurse licensed in the state of Texas
- Must have minimum 1 year recent nursing practice experience in Medicare home health visits
- Able to communicate timely and effectively for patient issues
- Able to utilize mobile devices and Electronic Medical Records without delay.
- *Knowledge of OASIS assessment and Plan of Care creations and proper nursing documentation is a must*
- Ability to start an IV therapy
- Trained on wound vac
- ability to perform finger sticks and venipuncture on regular bases
- Job Types: Full-time, Part-time, PRN, Per diem
- Pay: $50.00 - $120.00 per hour
- Work Location: On the road
