Senior Haven is a residential care facility designed and staffed uniquely for residents who have failed placement elsewhere due to complex behaviors and/or medical needs. We are looking for a _*Full Time LPN Wellness Nurse*_ to work alongside our Director of Nursing to provide exceptional resident care. *Responsibilities*
- Working hand in hand with our Director of Admissions to conduct pre-Admission assessments/screenings.
- Provide oversight of resident's Service Plan most especially Quarterly.
- Evaluations and Significant Changes of Condition.
- Monitor and assess resident physical and cognitive functioning.
- Frequent communication with families and external care support teams.
- Coordination with the pharmacy, physician, resident and/or family to ensure accurate medication orders.
- Train, supervise, lead, and motivate the team to bring the highest quality of care to residents.
- Administration and documentation of medications and other treatments.
- Administer non-delegated tasks such as Intramuscular Injection.
- Other duties as Assigned.

- Must be available to work Thursday-Sunday, 4x10 schedule.
- Most importantly, have patience and understanding and a desire to work within behavioral health!
