Senior Haven is a residential care facility designed and staffed uniquely for residents who have failed placement elsewhere due to complex behaviors and/or medical needs. We are looking for a Full Time LPN Wellness Nurse to work alongside our Director of Nursing to provide exceptional resident care. We are looking for clinicians who would like to combine their love for people and strong work ethic with the opportunities to advance their career.

Responsibilities

Working hand in hand with our Director of Admissions to conduct pre-Admission assessments/screenings.
Provide oversight of resident's Service Plan most especially Quarterly.
Evaluations and Significant Changes of Condition.
Monitor and assess resident physical and cognitive functioning.
Frequent communication with families and external care support teams.
Coordination with the pharmacy, physician, resident and/or family to ensure accurate medication orders.
Train, supervise, lead, and motivate the team to bring the highest quality of care to residents.
Administration and documentation of medications and other treatments.
Administer non-delegated tasks such as Intramuscular Injection.
Other duties as Assigned.
