About this role
As a Navigator RN, you will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys. Coordinate patient care across the continuum from diagnosis through treatment, follow-up, and transition from care. Assess patient and family needs, provide education on diagnoses and treatment options, and offer ongoing clinical, emotional, and psychosocial support.
Collaborate with physicians, multidisciplinary teams, and community resources to develop individualized care plans. Advocate for patients and families by promoting informed decision-making. Monitor patient progress, document assessments, education, and interventions.
Support quality improvement and departmental initiatives.
