Check out our website: Home - Fortis About the Role We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide high-quality, compassionate in-home nursing care. This role supports patients’ health, safety, and daily care needs as part of an interdisciplinary team. Responsibilities Provide skilled nursing care per physician orders and care plans Monitor patient condition, assess pain, and educate patients and families Administer medications safely following the 7 rights Ensure patient safety and maintain confidentiality Communicate patient status and needs with the care team Complete accurate and timely documentation Participate in required trainings, in-services, and meetings Follow agency policies, procedures, and emergency protocols Disclaimer: This description reflects the general nature and level of work for the Licensed Practical Nurse (LPN) position. Graduate of an accredited nursing program Active LPN license in the state of Indiana Recent acute care or home health experience (home health preferred) Valid driver’s license, reliable vehicle, and auto insurance Current TB test and CPR certification
