Position Summary
We are seeking a compassionate and experienced Registered Nurse (RN) / Care Manager to join our office-based primary care team. The ideal candidate will have experience in clinical care coordination, patient education, and managing both acute and chronic health conditions in an outpatient setting.
Main Responsibilities
- Provide patient care and medical triage within the primary care office setting
- Coordinate care across multiple providers and services
- Educate patients and families on disease management, preventive care, and treatment plans
- Administer vaccines and assist with office-based procedures
- Support patients with behavioral health, substance use, and chronic disease management (such as diabetes)
- Contribute to a collaborative team environment focused on quality care and patient satisfaction
Qualifications
Current Registered Nurse (RN)* license (required)
- Experience in a primary care or outpatient setting preferred
- Strong communication and teaching skills
- Ability to multitask and work collaboratively within a care team
- Experience with care management, behavioral health, substance use, or diabetes education a plus
