Day to day: leading a clinical team Registered Nurse (RN) - Clinical Team Leader - Home Health & Hospice
Day to day: approving start-of-care and recertification patient information Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit.
Day to day: coordinating home health and hospice services for each patient Ensure the overall coordination of home health & hospice services provided to the patient is delivered according to acceptable standards of practice and company procedures.
Day to day: liaising between physicians, referral sources, patients and caregivers Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees.
Cross-training: working in both home health and hospice You’ll work in both care settings, giving you a broader clinical experience and the chance to support patients and families throughout their full care journey.
