Wesley Gardens is actively seeking motivated Licensed Practical Nurses for our community in Rochester, NY. Our Licensed Practical Nurses assist in ensuring the general health and well-being of our residents by providing nursing care.

LPN Responsibilites

Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.

Provide regular patient status updates.

Assist in implementing patient’s treatment plan under direction of the Registered Nurse.

Complete required documentation in an accurate and timely manner.

Supervise, administer and or assist with medications.

Provide direct care of residents as necessary.

Follow the plan of care for each resident.

Ensure the residents care standards are maintained.
