We’re looking for a compassionate and skilled Registered Nurse (RN) who thrives in a more personal, patient-focused setting. In this role, you’ll have the autonomy to manage your own patients, build meaningful relationships, and deliver high-quality care in the home—whether supporting recovery or providing comfort-focused hospice care.
What You’ll Do
- Provide skilled nursing care, assessments, and treatments in patients’ homes
- Develop and manage individualized care plans for home health and/or hospice patients
- Monitor patient progress and adjust care as needed
- Collaborate with physicians, caregivers, and the interdisciplinary team
- Educate patients and families on care, medications, and disease progression
- Complete timely and accurate clinical documentation

What You Bring
- Active RN license (state of practice)
- 1+ year of clinical experience (home health, med/surg, or critical care preferred)
- Strong clinical judgment and communication skills
- Ability to work independently and stay organized
- Valid driver’s license and reliable transportation
