LPN - Primary Care - Pagosa Spring Integrated Healthcare – Pagosa Spring, CO

Position Summary

We are seeking a LPN who provides direct nursing services for clinic patients and clinical and administrative support to medical providers. Nursing duties include preparing for patient visits, obtaining and documenting vital signs, assisting medical providers with procedures, communicating with patients, including providing health education, chronic care management, preventative care, and immunizations for patients across the lifespan. Assigned to a primary location, but may be required to assist at other clinics.

Salary: Starting pay is $28.13 per hour and ranges up to $32.32 per hour and considers preferred experience and preferred education described above.

Physical demands

Employee must occasionally lift and/or move 15 pounds up to 50 pounds on an occasional basis. Employees are frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Must have the ability to sit for longer periods at a computer. Employees must be able to travel between AHS locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. Duties

Provides nursing direct care (acute and chronic) across the lifespan from pediatrics to geriatric nursing care. This includes but is not limited to care coordination, direct nursing intervention, medication/vaccine administration, treatments, sterilization, testing, procedures, phlebotomy, patient education and wound care. Regularly assists providers with chronic care management and care plan updates for patients who have complex needs. Maintains knowledge of the uses and effects of medications and immunizations, including long acting injectable medication. Completes all clinical documentation within expected timeframes

Timely completion of assigned E-Learning Courses: Employee has completed all required essential learning courses by the due date. Supports clinic workflow by monitoring schedules, preparing for visits, assisting with urgent orders and referrals, and delegating tasks to the Medical Assistants.
