Position
Summary
We are seeking a LPN who provides direct nursing services for clinic patients and clinical and administrative support to medical providers. Nursing duties include preparing for patient visits, obtaining and documenting vital signs, assisting medical providers with procedures, communicating with patients, including providing health education, chronic care management, preventative care, and immunizations for patients across the lifespan. Assigned to a primary location, but may be required to assist at other clinics.

Duties
Provides nursing direct care (acute and chronic) across the lifespan from pediatrics to geriatric nursing care. This includes but is not limited to care coordination, direct nursing intervention, medication/vaccine administration, treatments, sterilization, testing, procedures, phlebotomy, patient education and wound care. Regularly assists providers with chronic care management and care plan updates for patients who have complex needs. Maintains knowledge of the uses and effects of medications and immunizations, including long acting injectable medication. Completes all clinical documentation within expected timeframes

Communication and Customer Service: Committed to prompt and clear communication in service to both internal and external customers by returning phone calls and e-mails quickly and ensuring response within two business days unless extenuating circumstances apply.
Teamwork: Actively seeks and provides guidance, and is a contributor to the organization and any teams or committees they are involved in. The employee is respectful, kind and helpful when working with other staff. Training: Attends all mandatory staff meetings and trainings.
Attendance/Punctuality: Employee arrives at work on time, follows the required call-in procedures for attendance and attends meetings on time. Timely completion of assigned E-Learning Courses: Employee has completed all required essential learning courses by the due date. Supports clinic workflow by monitoring schedules, preparing for visits, assisting with urgent orders and referrals, and delegating tasks to the Medical Assistants.

Physical demands
Employee must occasionally lift and/or move 15 pounds up to 50 pounds on an occasional basis.
