*Job Summary*
Harmoni Care Solutions is seeking a dedicated and compassionate *Licensed Practical Nurse (LPN) *to provide In-Home Nursing Respite (IHNR) services to a client in Jackson, MS. This is a Full-Time position requiring *Sunday - Saturday 10:00am - 4:00pm*, providing skilled nursing support and personal care in the client's home.
Responsibilities
- Provide total assistance with bathing, personal hygiene, and grooming needs, as client is unable to participate independently
- Provide total assistance with mobility, including transfers, positioning, turning/repositioning, and use of proper body mechanics and/or lift equipment to prevent injury
- Assist with feeding and mealtime support
- Implement seizure precautions per care plan, including environmental safety checks, safe positioning before/during/after a seizure event, timing and documenting seizure activity, administering rescue protocol/medication if ordered, and post-ictal monitoring
- Administer medications per physician orders and care plan
- Monitor and document client's health status, vital signs, and any changes in condition
- Report changes in client condition to supervising RN and/or care team promptly
- Maintain accurate and timely documentation of services provided
- Follow the individual's approved care plan and Harmoni Care Solutions' policies and procedures
- Ensure a safe, clean, and supportive home care environment
- Communicate professionally with client, family members, and care team
*Requirements*
- Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Mississippi
- Current CPR certification (or willingness to obtain)
- Valid driver's license
- Valid automobile insurance
- Reliable transportation to and from Carthage, MS
- Current TB skin test
- Current physical
- Background clearance (via MS Department of Health)
- Prior home health, private duty, or IDD Waiver experience preferred
- Strong communication, documentation, and interpersonal skills
- Compassionate, patient-centered approach to care
