Senior Planning Group is seeking a compassionate, experienced, and highly organized Certified Nurse Case Manager to join our growing team. This is a part-time, fully remote position serving clients throughout Waukesha, Washington, Milwaukee, and Ozaukee Counties. The ideal candidate is a Registered Nurse with an active Certified Case Manager (CCM) credential who is passionate about helping seniors and their families navigate healthcare, long-term care, and community resources. Essential duties include conducting comprehensive client assessments, developing and monitoring individualized care plans, coordinating care with physicians/providers/caregivers/family members, providing medication administration and medication management oversight, monitoring client progress, educating clients and families, assisting with Medicare/Medicaid/long-term care navigation, maintaining documentation, and advocating for clients.

Requirements

Current Wisconsin RN license in good standing
Current Wisconsin RN license in good standing
Certified Case Manager (CCM) certification required
Minimum of two years of case management experience
Experience working with seniors preferred
Strong knowledge of Medicare, Medicaid, long-term care services, and community resources
Excellent communication, organization, and problem-solving skills
Ability to work independently in a remote setting
