Senior Planning Group is seeking a compassionate, experienced, and highly organized Certified Nurse Case Manager to join our growing team. This is a part-time, fully remote position serving clients throughout Waukesha, Washington, Milwaukee, and Ozaukee Counties. The ideal candidate is a Registered Nurse with an active Certified Case Manager (CCM) credential who is passionate about helping seniors and their families navigate healthcare, long-term care, and community resources.

Essential duties include conducting comprehensive client assessments, developing and monitoring individualized care plans, coordinating care with physicians/providers/caregivers/family members, providing medication administration and medication management oversight, monitoring client progress, educating clients and families, assisting with Medicare/Medicaid/long-term care navigation, maintaining documentation, and advocating for clients.
