RN Health Care Supervisor - Day Habilitation

ws a minimum of every three months, monitoring and recording vital signs and weights every six months, contacting family/community residences physicians other health care professionals and service coordinators as necessary, participating in DHSP meetings, acting as a member of the Interdisciplinary Team, screening admission applications for medical information, monitoring the general health and well-being of each consumer on a daily basis, obtaining and maintaining medical records, developing and maintaining nursing care plans, documenting nursing notes on monthly and quarterly progress notes, maintaining consumer file, adhering to all medical and funding source regulations and Venture policies and procedures, adhering to all medical requirements from accreditation agency, educating consumers in hygiene and health concerns, oversight of maintenance-therapy treatment as recommended by a Therapist, providing and coordinating medical/health related in-service training to employees including Universal Precautions and Venture's Bloodborne Pathogen Exposure Control Plan, acquiring and maintaining certification to become an agency MAP trainer, and availability to provide at least two agency MAP training sessions per year and participate in staff MAP recertification testing.
