*Position Overview* A leading healthcare organization is seeking a compassionate and experienced Registered Nurse Case Manager to provide hospice care services to patients in their homes throughout Monmouth County. This role is responsible for coordinating and delivering comprehensive nursing care, ensuring quality outcomes, regulatory compliance, and effective communication among patients, families, physicians, and interdisciplinary care teams.
*Schedule*
- Full-Time, 40 hours per week
- Day Shift: 8:30 AM - 5:00 PM
- Every fourth weekend required
- Holiday rotation: 2 of 6 holidays

*Key Responsibilities*
- Conduct comprehensive patient assessments and develop individualized care plans.
- Coordinate and manage hospice nursing services in accordance with physician orders and regulatory requirements.
- Monitor patient progress and revise care plans as clinical needs change.
- Complete medication reconciliation and collaborate with physicians regarding treatment plans.
- Provide patient and caregiver education to support care goals and symptom management.
- Document assessments, interventions, care coordination activities, and clinical decisions accurately and within required timelines.
- Ensure visit frequency and authorizations align with payer guidelines and approved plans of care.
- Serve as a liaison between patients, families, physicians, and interdisciplinary team members.
- Supervise home health aide staff and complete required supervisory visits.
- Participate in care transitions and maintain continuity of services.
- Maintain productivity standards and support quality patient outcomes.
- Follow organizational policies, nursing standards, and ethical guidelines.
*Required Qualifications*
- Associate Degree in Nursing or Diploma in Nursing.
- Minimum of six (6) months of RN experience, or at least one (1) year of LPN experience.
- Home health, hospice, or related community-based nursing experience preferred.
- Basic computer proficiency and documentation skills.
- Active New Jersey Registered Nurse (RN) license.
- Current AHA Basic Life Support (BLS) for Healthcare Providers certification.

- Bachelor of Science in Nursing (BSN).
