About this role
The Home Health Nurse is responsible for the delivery of coordinated nursing care to patients in their place of residence. Implements and evaluates a comprehensive, goal focused care plan in partnership with the patient, representative (if applicable) and caregivers as developed and approved by the physician.
Qualifications
Graduate of an accredited school of nursing, Baccalaureate degree preferred Current licensure as a registered nurse in the State of Illinois. Minimum of two years of nursing in an acute care or home care setting preferred CPR certification preferred, not required Current Illinois driver’s license with proof of automobile insurance on personal vehicle for business use or alternate reliable transportation Must be empathetic, non-judgmental, tactful, responsible, adaptable, organized, and committed to the purpose and objectives of the organization Must have excellent communications skills. Must have emotional and mental maturity necessary for establishing and maintaining good working relationships with patients, patient’s families, and Agency personnel.
Must understand the team approach and be a supportive team member.
