About this role
Registered Psych Nurse (RN) - Home Health. Administer nursing care for clients in their residence, coordinate care with the interdisciplinary team, clients and their families, and the referring agency. Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health clients.
Regularly reevaluate the client's nursing needs and evaluate the outcomes of care. Develop, initiate, and revise the Plan of Care (POC). Provide effective communication to clients, families, team members, and other health care professionals.
Apply infection control and universal precaution. Coordinate services and inform physician of changes. Complete appropriate documentation.
Supervise, teach, and provide clinical direction to other nursing personnel. Perform on-call responsibilities.

Requirements
- RN license
- Two (2) year experience as an RN in a psychiatric clinical care setting
- Home health experience (preferred)
- Psychiatric-Mental Health Nursing Certification (preferred but not required)
- Completion of an accredited Registered Nursing Program
- BSN (preferred)
- Current driver's license in good standing
- Reliable transportation and auto liability insurance
- Current State professional license as an RN
