Under the supervision of the Director of Nursing or RN, the LPN is responsible for providing care services in accordance with specific physician orders, state and federal guidelines, and the department’s policies and procedures.

Job Duties

Follows established procedure for charting and incident reporting.

Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.

Documents any change in patient conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.

Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.

Document and notify applicable departments of any patient room changes, LOA, new admits, or discharges.

Makes rounds with physicians when necessary

Experience

Current LPN Licensure.

Current ACLS and BLS Certification

Must be able to fluently read, write, and speak English

Represents the organization in a positive and professional manner

Actively participates in performance improvement and continuous quality improvement (CQI) activities

Communicates and complies with the Sweetwater Mission, Vision, and Values as well as the focus statement of the department

Complies with State and Federal Guidelines, along with Organizational Policies and Procedures

Complies with Health and Safety Standards and Guidelines

Must possess a sincere desire to serve the ill, aged, and infirm.

Ability to read, write and speak the English language.

Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the public.

Willingness to work harmoniously with other personnel.

Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level; they are currently functioning.

Must be in good general health and demonstrate emotional stability.

Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
