About this role

We are seeking a dedicated RN/Wellness Director to oversee and manage the wellness program at our facility. Duties include creating and maintaining 24-hour Plan, Service Plan, Quarterly Plan and RAI, and TB; meeting quarterly with residents to evaluate wellness; reviewing service plans and C. N. A compliance; meeting all Supportive Living requirements; reviewing and signing all necessary Supportive Living paperwork; meeting weekly with Executive Director and Wellness Coordinator; recording and monitoring all Progress Notes; coordinating with resident's home health, physicians and Eden on-site Clinic; recording, filing and maintaining resident Bed Hold; performing all hospital follow-ups and facilitating resident return; creating, reviewing and filing all Medication Error Reports; addressing Supportive Living code issues; meeting with residents to discuss health concerns and health promotion; creating and/or reviewing Incident Reports; assisting in initial interview process; upon intake, interviewing resident and completing 24-hour Assessment; reviewing medications; overseeing ordering, set-up and administration of resident's medication; ensuring residents receive proper supervision or assistance; training and supervising Licensed Practitioner Nurse; checking all pharmacy deliveries for completeness and accuracy; overall responsibility for all Wellness Staff including providing initial training of LPN and Wellness Coordinator and continuing education; responsible for LPN schedule; monitoring LPN, Wellness Coordinator and CNA Database entries.
