Pine Haven Nursing & Rehabilitation Center, a leading healthcare provider, is seeking a skilled and compassionate RN Registered Nurse Care Plan Coordinator to join our team in Philmont, New York. As a key member of our care coordination team, you will play a vital role in developing and implementing individualized care plans for our residents, promoting their optimal health and well-being. Responsibilities include developing, implementing, and evaluating individualized care plans for residents, conducting comprehensive assessments, utilizing electronic medical records to document care plans, coordinating care conferences and family meetings, collaborating with healthcare providers, monitoring and reporting changes in resident condition, and participating in quality improvement initiatives.
Requirements
RN license in good standing, with current registration in the State of New York; prior experience as a registered nurse, with a strong foundation in care coordination and patient-centered care; excellent communication, interpersonal, and organizational skills; ability to maintain confidentiality and handle sensitive information with discretion.
