We are looking for a Full OR Part Time RN. _*You decide your case load!*_ We have several coverage areas to choose from - West St. Louis County, St. Charles, Wentzville, etc.
*Job Responsibilities*
- Manage a caseload of patients; work with the patient, their family, and their physicians to determine how often and when to make skilled nursing visits;

- Coordinate with patients and physicians to develop a plan of care;
- Provide nursing care, education, and treatments as ordered;
- Work with PT, OT, ST, and Social Work to provide for patients’ needs; and
- Perform and complete Start of Care, Recertification, Resumption of Care, and Discharge Comprehensive OASIS assessments.
