Licensed Practical Nurse - Home Health Nurse

We are hiring Licensed Practical Nurses (LPNs) for a home health assignment in Virginia Beach, VA.

The LPN will provide skilled nursing care to patients in their homes and will work collaboratively with Registered Nurses and other members of the healthcare team.

Candidates should have experience in home health, wound care, wound vac, IV therapy, phlebotomy/lab draws, and chronic disease management.

Responsibilities

• Provide direct and indirect patient care in the home setting.
• Follow the patient's individualized plan of care.
• Perform skilled nursing treatments within the LPN's scope of practice.
• Provide wound care and wound vac treatment.
• Perform IV therapy and infusion-related services as ordered.
• Perform lab draws and phlebotomy.
• Provide ostomy and catheter care.
• Assist with chronic disease management.
• Monitor and document patient status and progress.
• Educate patients and caregivers on treatment plans and disease management.
• Communicate patient condition and changes to the supervising RN/healthcare team.
• Maintain accurate patient documentation in accordance with home health requirements.
• Provide professional, compassionate, patient-centered care.

Required Qualifications

Active Virginia LPN license – REQUIRED*

• Home health experience preferred.
• Wound care experience – required/preferred based on assignment.
• Wound vac experience.
• IV therapy experience.
• Lab draw/phlebotomy experience.
• Chronic disease management experience.
• HCHB (Homecare Homebase) experience preferred.
• Excellent communication and bedside manner.
• Ability to work independently in patients' homes.
• Reliable transportation.
• Ability to travel within the assigned service area.

• Pay: $35.00 - $40.00 per hour
• Expected hours: 40.0 per week
• Work Location: In person
