Responsibilities

Participates in the delivery of care as set by the care plan created by the RN Case Manager and the interdisciplinary team.

Assists with planning and implementing ongoing audit reimbursement program in accordance with federal, state, and Hospice standards, under the supervision of a registered nurse.

Willingness to be flexible and agile as a field LPN.

Reports findings and concerns to the RN Case Manager.

Audits one complete chart for each field nurse in the QAPI team monthly.

Possesses adequate knowledge of Medicare/Medicaid hospice benefit and insurance standards to ensure reimbursement for all services rendered and the regulatory standard of care is provided.

Makes visits as scheduled

Maintains patient rights and dignity. Assures the care of the dying patient, optimizing his/her comfort and dignity.

Successful completion of LPN or LVN training program

Experience

1 year related

Previous hospice or home health experience a plus

Current LPN license; driver’s license and current automobile liability insurance
