Responsibilities
- Assess patients' health conditions and develop individualized care plans in collaboration with physicians and family members
- Administer medications, treatments, and injections as prescribed
- Monitor and document patients' progress, reporting any changes or concerns to the healthcare team
- Educate patients and their families on health management, medication usage, and safety procedures
- Coordinate with other healthcare providers to ensure comprehensive patient care
- Maintain accurate and timely documentation in compliance with healthcare regulations and standards
- Adhere to infection control protocols and safety guidelines to ensure a safe environment for patients and staff
Skills and
Qualifications
- Valid Registered Nurse (RN) license in the state of practice
- Proven experience in home health care or related clinical settings preferred
- Strong assessment, critical thinking, and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
- Knowledge of healthcare regulations, safety standards, and documentation requirements

Educational/Degree: Graduate from an accredited school of nursing
Training/License: Currently licensed as a registered nurse to practice in the state.
Physical
Requirements
Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations.

Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability. Must be knowledgeable of Hospice regulations, guidelines, LCD criteria, and where to find this information.
