RN Case Manager/Supervisor

A Plus Home Care, Inc. is searching for a dependable, dedicated and hard-working Registered Nurse for our RN Nurse Supervisor position that is excited about being challenged and can adapt and flex to a growing, changing company. The position has flexible hours ranging from 15-30 hours per week depending on your schedule and availability. We offer flexible hours, the ability to create your own schedule, and an option to do some work remotely from a computer. Duties include conducting assessments of client needs, oversight and supervision of field staff, meeting with new and existing clients, creation of client Care Plans, training of field staff, charting of progress notes, collaboration and care coordination, review of field staff competency, and being a resource for field staff.
