Hospice Admissions Registered Nurse (RN)

This is more than completing an admission.

You will often be the first nurse a patient and family meets. You will help them understand hospice, answer difficult questions, create trust, and make sure their transition into care feels organized, personal, and supported from the start.

We are looking for someone who already understands hospice and is ready for a role with more impact, more ownership, and room to grow.

What You’ll Do

As a Hospice Admissions RN, you will complete admissions in patient homes, hospitals, skilled nursing facilities, assisted living communities, and other care settings throughout Deerfield, Skokie, North Chicago, Northwest Chicago, and surrounding communities .

You will assess patients for hospice eligibility, complete clinical documentation, coordinate physician orders, communicate with the interdisciplinary team, and educate patients and families about hospice services, goals of care, and what to expect next.

Most importantly, you will help families feel informed, respected, and supported during a difficult time.

What We Offer

• $90,000–$105,000 annually, based on hospice experience and qualifications, with full-time benefits eligibility

• Mileage and eligible toll reimbursement for work-related travel

What We’re Looking For

• Active Illinois RN license with hospice experience required ; hospice admissions experience strongly preferred

• Strong clinical assessment, hospice eligibility, documentation, and goals-of-care communication skills

• Ability to work independently while communicating effectively with patients, families, physicians, facilities, and the interdisciplinary team

• Reliable transportation, valid driver’s license, automobile insurance, and ability to travel throughout the assigned service area
