Home Physicians Group is seeking a Primary Care Physician practicing in Skilled Nursing, Assisted Living and Independent Living facilities for a Provider run group in Central Florida. We are a large practice with over 25 practitioners that serve the skilled nursing facilities, assisted living facilities and our homebound patients in the comfort of their home in 14 counties. As an Advanced Practice Registered Nurse (APRN), you will play a crucial role in delivering high-quality healthcare services to patients in various settings depending on the location and care to be given. Your primary responsibility will be to assess, diagnose, and manage patient care, ensuring that each individual receives personalized treatment plans tailored to their unique needs. You will collaborate with a multidisciplinary team to enhance patient outcomes and promote health education within the community.

This position requires a strong commitment to patient advocacy and the ability to navigate complex healthcare systems effectively. Ultimately, your work will contribute to improving the overall health and well-being of the population you serve.

Minimum

Qualifications

Master's degree in Nursing from an accredited program. Current APRN licensure in the state of SMPL. Certification in a relevant specialty area.

Experience in a clinical setting, particularly in primary care or specialty areas. Bilingual skills to enhance communication with diverse patient populations. Familiarity with electronic health record (EHR) systems.

Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans. Prescribe medications and treatments in accordance with established protocols. Monitor patient progress and adjust treatment plans as necessary. Educate patients and their families about health management and disease prevention. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
