We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the direction of a Registered Nurse (RN).

Position Summary
The LPN provides skilled nursing services in accordance with physician orders, established care plans, RN direction, agency policies, and applicable state and federal regulations.
Responsibilities
Provide skilled nursing care to patients in the home setting Administer medications and treatments within LPN scope of practice Perform wound care, dressing changes, catheter care, and other skilled nursing tasks Monitor patient condition and report changes to the supervising RN Reinforce patient and caregiver education Follow established plans of care and physician orders Complete accurate and timely documentation in the EMR Communicate with RNs, physicians, and other members of the care team Participate in required training, meetings, and in-services Provide care in accordance with Medicare requirements, state regulations, and agency policies
Qualifications
- Graduate of an accredited Practical Nursing program
- Current Florida LPN license in good standing
- Minimum 1 year of nursing experience preferred
- Home health, hospice, or acute care experience preferred
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Ability to meet all required background and compliance requirements
