Hospice Registered Nurse

• Conduct thorough patient assessments to evaluate symptoms, pain levels, and overall condition, ensuring personalized care plans are tailored to each individual’s needs.
• Administer medications and treatments in accordance with physician orders, monitoring for effectiveness and side effects.
• Collaborate with interdisciplinary teams—including physicians, social workers, chaplains, and therapists—to coordinate holistic end-of-life care.
• Educate patients and families on disease progression, symptom management, medication usage, and advanced directives to promote understanding and comfort.
• Provide emotional support and counseling to patients and families navigating complex end-of-life decisions while respecting cultural and personal preferences.
• Document all patient interactions accurately in medical records while maintaining compliance with HIPAA regulations.
• Facilitate discharge planning by coordinating services such as home health care or inpatient transfers when necessary.

*Experience*

• Prior experience in critical care settings such as ICU, Emergency Room, or Level I/II Trauma Centers demonstrating advanced patient assessment skills.
• Proven background in hospital medicine or post-acute care environments with exposure to medical-surgical units or outpatient clinics.
• Familiarity with infectious disease care, geriatrics, behavioral health, pediatrics (PICU), or OR experience is highly desirable.
• Strong knowledge of physiology, medical terminology, diagnostic evaluation, utilization management, case management, and discharge planning processes.
• Experience working within managed care systems and handling complex patient service scenarios is advantageous.
• Demonstrated ability to perform comprehensive vital signs assessment and manage acute care situations effectively.
• Excellent communication skills for intake procedures, medical records documentation, patient education, and interdisciplinary collaboration. Pay: $35.00 per hour Work Location: In person
