What you will do

The Licensed Vocational Nurse (LVN) will assist the Advanced Practice Provider (NP/PA) in managing assigned patient case load. The LVN will provide direct patient care within scope of practice, perform follow-up assessments, gather relevant health/social information as delegated by provider, and coordinate care to implement treatment plan.

Required

Education

Graduate of an accredited school of nursing Required

Skills

Nursing skills as defined in Nursing Standards of practice. Basic computer skills.

Required Licensure/Registration/

Certification

Current Licensed Vocational Nurse in state of Texas; CPR; Texas Driver’s License; Valid Auto Liability Insurance. Must have reliable transportation.

Required

Experience

Minimum of two years clinical nursing experience.

Preferred

Experience

Hospice/Palliative or case management experience.
