Position Summary

We are seeking compassionate, dependable, and clinically skilled Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our skilled nursing facility team. Nurses are responsible for providing safe, high-quality, resident-centered nursing care to residents with a variety of medical, rehabilitative, and long-term care needs.

Essential Responsibilities

  • Provide direct nursing care to residents in accordance with physician orders, care plans, facility policies, and applicable regulations.
  • Complete comprehensive nursing assessments and monitor residents for changes in condition.
  • Administer medications and treatments accurately and safely.
  • Monitor vital signs, blood glucose levels, wounds, pain, nutrition, hydration, and other clinical conditions as appropriate.
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
  • Recognize changes in residents' conditions and promptly notify the appropriate provider, supervisor, and/or responsible party.
  • Respond appropriately to emergencies and initiate interventions within the nurse's scope of practice.
  • Perform treatments and procedures according to established standards and physician orders.
  • Document assessments, interventions, medications, treatments, and resident responses accurately and timely.
  • Communicate effectively with physicians, nurse practitioners, CNAs, rehabilitation staff, dietary staff, social services, residents, and families.
  • Supervise and provide direction to Certified Nursing Assistants (CNAs) and other nursing staff as appropriate.
  • Participate in shift-to-shift report and ensure continuity of care.
  • Assist with admissions, transfers, discharges, and hospital returns.
  • Maintain accurate clinical records and complete required documentation.
  • Follow infection prevention and control practices, including appropriate use of personal protective equipment.
  • Maintain resident dignity, privacy, safety, and confidentiality at all times.
  • Provide compassionate support to residents and their families.
  • Participate in quality assurance, performance improvement, and facility education programs.
  • Follow all applicable federal, state, and local regulations governing skilled nursing facilities.

Qualifications

RN

  • Current, unrestricted Registered Nurse license in the applicable state.
  • Graduate of an accredited nursing program.
  • Current CPR/BLS certification or ability to obtain as required.
  • Previous skilled nursing, long-term care, rehabilitation, or geriatric nursing experience preferred.

LPN

  • Current, unrestricted Licensed Practical Nurse license in the applicable state.
  • Graduate of an approved practical nursing program.
  • Current CPR/BLS certification or ability to obtain as required.
  • Previous skilled nursing, long-term care, rehabilitation, or geriatric nursing experience preferred.

Physical Requirements

  • Ability to stand, walk, bend, reach, and move throughout the facility for extended periods.
  • Ability to assist with resident care and repositioning in accordance with safe-lifting practices and facility procedures.
  • Ability to respond promptly to resident needs and emergencies.
  • Ability to communicate effectively with residents, families, and staff.
