Responsible for accurately collecting and entering patient medication histories into the hospital's information system for utilization by health care professionals during the hospital encounter. Duties include collecting patient home medication lists, gathering information from various resources, communicating with health care professionals, entering medication lists into the hospital's system, directing unresolved issues to the Pharmacist, administering defined medications and vaccinations, performing screening assessments for drug and alcohol, suicide, Hep C, pneumococcal and influenza, and placing orders for medication, lab and support services.

Requirements

  • High school diploma or GED required
  • LPN or RN licensure; or Pharmacist with vaccine licensure (immunization certification through APhA)
  • Licensed Practical Nurse (Required)
  • Registered Nurse - PA (Required)
  • Immunization Approval (Required)
  • Must obtain 80% or higher on department administered exam on medication history process
  • General computer skills
  • Considerable knowledge of medication names (trade and generic), uses, doses, and routes of administration
