Requirements
- High school diploma or GED required
- LPN or RN licensure; or Pharmacist with vaccine licensure (immunization certification through APhA)
- Licensed Practical Nurse (Required)
- Registered Nurse - PA (Required)
- Immunization Approval (Required)
- Must obtain 80% or higher on department administered exam on medication history process
- General computer skills
- Considerable knowledge of medication names (trade and generic), uses, doses, and routes of administration
