As a Home Care Nurse at Health Care of South Florida, you will play a vital role in delivering high-quality nursing care to patients in their homes. This position is essential to our mission of providing compassionate and personalized healthcare services, particularly for those requiring specialized post-transplant and rehabilitation care. Responsibilities: • Provide skilled nursing care to patients in their homes, ensuring adherence to individualized care plans. • Administer medications and perform medical procedures such as IV infusion and venipuncture as required. • Monitor patient progress and report changes in condition to the healthcare team. • Educate patients and families on health management and post-care instructions. • Collaborate with interdisciplinary teams to coordinate comprehensive patient care. • Maintain accurate documentation of patient assessments, interventions, and outcomes. • Ensure compliance with all health regulations and agency policies. Qualifications: • Current RN license in Florida with relevant home health experience preferred. • Strong knowledge of anatomy and medical-surgical nursing practices. • Proficiency in IV infusion techniques and venipuncture procedures. • Experience caring for infants or children is a plus but not mandatory. • Excellent communication skills for effective patient interaction and education. • Ability to work independently while managing multiple patient cases effectively. • Compassionate demeanor with a strong commitment to patient-centered care. Pay: $30.00 - $75.00 per hour Work Location: In person
