Family Nurse Practitioner/ Physician Assistant

We are seeking a Nurse Practitioner or Physician Assistant to join our team caring for patients in skilled nursing and long-term care facilities in the Tri-Cities area.

This position is PRN.

Call coverage is provided by a national telehealth group, with the NP/PA available for back-up and oversight of any hospital transfers.

Experience in post-acute or long-term care (PA/LTC) is preferred, but providers with other relevant experience are encouraged to apply.

Key Responsibilities

• Serve as the primary care provider for patients in skilled nursing and long-term care settings, ensuring high-quality, patient-centered care.
• Perform comprehensive patient assessments and evaluations to develop individualized treatment plans aligned with each patient’s goals.
• Diagnose and manage acute and chronic conditions, coordinating timely interventions to promote recovery and prevent unnecessary hospitalizations.
• Accurately document all patient encounters and care plans in medical records, including appropriate ICD-10 coding, to support continuity of care and compliance.
• Partner closely with nursing staff, facility teams, and other healthcare professionals to deliver coordinated, compassionate care.
• Educate patients and families about health conditions, treatment options, and preventive care, fostering understanding and shared decision-making.
• Complete required Medicare recertification visits and documentation to ensure uninterrupted patient services and regulatory compliance.

Qualifications

• Master’s or Doctoral degree in Nursing (MSN or DNP) or Master's of Physician Assistant Studies (MPAS).
• National board certification (AANP, ANCC, or NCCPA) in applicable specialty.
• Current, active license in the state of Tennessee.
• Current DEA registration (or eligibility to obtain).
