Registered Nurse (RN) – Home Care Assessment & Care Plan Development. SYNERGY HomeCare of Mt. Laurel is seeking a compassionate and detail-oriented Registered Nurse (RN) to conduct non-medical client assessments and develop individualized care plans for clients receiving home care services. Responsibilities include conducting in-home assessments, evaluating physical/cognitive/emotional/environmental and safety needs, developing individualized care plans, reviewing medications, collaborating with clients/families/caregivers/physicians/hospice providers, conducting supervisory visits, ensuring compliance with New Jersey regulations, and completing accurate documentation.

Requirements
- Active New Jersey Registered Nurse (RN) license in good standing
- Active New Jersey Registered Nurse (RN) license in good standing
- Minimum of one year of nursing experience preferred
- Home care, hospice, geriatrics, case management, or community health experience preferred
- Strong assessment, documentation, and communication skills
- Ability to work independently and manage a flexible schedule
