What You’ll Do
- Conduct wellness assessments and monitor resident health changes • Support medication management and perform MAR/TAR audits • Communicate with families, physicians and care teams • Monitor incidents, falls and follow-up care • Maintain documentation and compliance with regulations • Support infection control and clinical quality initiatives • Train and support care team members What You’ll Bring: • Active LPN/LVN license • 1+ year experience in senior living or clinical setting • Strong assessment and documentation skills • Knowledge of medication administration and compliance • Strong communication and problem-solving skills Wellness Nurse (LPN/LVN) – Support Resident Health and Ensure Clinical Quality LPN | LVN | Wellness Nurse | Assisted Living | Senior Living | Medication Management | Resident Care
Pay Range
42.40 - 56.60
