Job description

Syntrillo is a digital health company that combines wearables and telemedicine to provide the next generation of neurological care. Our primary focus is secondary stroke prevention but we also care for patients with migraine, cognitive impairment, and other stroke risk factors.

The LPN will support clinical operations by making sure every patient has a complete chart, an accurate medication list, and the orders, referrals, and authorizations they need are completed in a timely manner. The work is detail-driven and largely asynchronous, done in the EHR, by phone, and by fax with outside offices, payers, and patients.

As patient volume grows, workflows continue to evolve. Syntrillo is looking for someone who can adapt processes to meet the needs of the clinic and keep them working as the practice scales.

This is a remote independent-contractor position open to candidates holding an active Virginia LPN license.

Responsibilities

*

● Chart review and prep — review patient records ahead of visits and prepare charts so providers have a complete clinical picture

● Outside records — request and retrieve records from outside offices, hospitals, and health systems, and ensure they are filed to the chart

● Medication reconciliation — reconcile medication lists against patient report, outside records, and pharmacy data, and flag discrepancies to the provider

● Order processing — process provider orders, including labs, imaging, and diagnostic testing, and track them through to results

● Referral processing — process incoming and outgoing referrals, coordinate with receiving offices, and follow up until the loop is closed

● Pre-authorization — obtain and track prior authorizations for medications, imaging, and services, and follow up with payers on pending requests

● Scheduling — assist with patient scheduling as needed

● Escalate clinical concerns promptly to the appropriate provider

● Document recurring processes and workflows for continuity

What the role requires

● Active, unencumbered LPN license in Virginia (multistate/compact license accepted)

● A minimum of one year of LPN experience

● Hands-on experience working in an electronic health record system

● Strong written and verbal communication, including the ability to follow up persistently and professionally with outside offices and payers

● Demonstrated organizational rigor and a reliable personal system for tracking outstanding work

● Sound judgment about when to research an answer independently and when to escalate immediately

● Comfort adapting to evolving workflows in a growing practice

● Based in the United States and authorized to work without sponsorship

● A private, HIPAA-appropriate workspace and stable broadband

● Willingness to complete a background check

● Two professional references, including one former direct manager

Strongly preferred

● Outpatient clinic or telehealth experience

● Experience with Epic, including retrieving outside records through Care Everywhere

● Prior authorization and referral coordination experience

● Neurology, stroke, cardiology, or primary care experience

● Experience in hypertension or chronic-disease management

● Prior remote work experience. Candidates without it who demonstrate exceptional organization and self-direction will be considered equally.

● Interest in telehealth, preventive care, or early-stage companies
