About this role
Accord Hospice of the White Mountains is seeking a compassionate and dedicated Hospice RN Case Manager. Responsibilities include assessing patients' physical, emotional, and psychosocial needs; educating patients, families, and caregivers on disease progression and care plans; managing pain and symptom control; coordinating care as the primary case manager; and collaborating with an interdisciplinary team. Monday–Friday schedule, typically 8:00 a.m. to 5:00 p.m. with flexible scheduling options.
