About this role

Job Type - PRN

The primary function of the Home Health RN is to provide, coordinate, and direct the provision of skilled nursing care and services to patients in their homes. The RN ensures quality and continuity of care by assessing patient needs, developing individualized care plans, performing OASIS assessments, coordinating with the interdisciplinary team, and educating patients and caregivers.

  • Current state license as a Registered Nurse (RN) in good standing and in accordance with the Nurse Licensure Compact, if applicable.
  • Minimum of one year of recent clinical experience as an RN in a home health setting .
  • OASIS assessment experience required .
  • Strong clinical and assessment skills with knowledge of home health regulations and guidelines (Medicare/Medicaid).
  • Excellent communication, organizational, and interpersonal skills.
  • Reliable transportation, valid driver’s license, and current auto liability insurance.
  • Must read, write, and comprehend English.

Environmental and Working Conditions

  • Provides care in patients’ homes under varied conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
  • Must be able to travel locally and work a flexible schedule.
  • Some exposure to weather and driving hazards.
  • Ability to handle on-call responsibilities as assigned.

Physical and Mental Effort

  • Requires prolonged standing, walking, and the ability to lift up to 50 lbs.
  • Requires working under pressure, meeting deadlines, and making clinical decisions quickly.
  • Requires manual dexterity, eye-hand coordination, and ability to manage stressful situations.

Essential Functions

  • Complete comprehensive OASIS assessments at admission, recertification, resumption of care, transfer, and discharge, accurately reflecting patient status.
  • Develop, implement, and update individualized patient care plans based on assessments and patient/caregiver input.
  • Provide skilled nursing interventions, including wound care, IV therapy, medication management, disease process teaching, and other clinical procedures as needed.
  • Educate patients and families on disease management, self-care, safety, and prevention strategies to promote independence.
  • Coordinate with physicians, therapists, home health aides, and other team members to ensure quality care.
  • Monitor patient progress, document timely and accurately, and ensure compliance with agency policies and payer requirements.
  • Provide supervision and direction to LVNs/LPNs and Home Health Aides.
  • Ensure infection control and adherence to standard precautions in all patient care activities.
  • Participate in quality assurance, performance improvement (QAPI) initiatives, and agency staff meetings.
  • Provide on-call services and emergency care as assigned.
  • Promote the agency’s philosophy and maintain professional growth and competency through in-service training.
