About this role

This nurse will provide skilled, compassionate care to hospice patients, support symptom management, monitor changes in condition, educate patients and families, and work closely with the interdisciplinary care team.

Position

Responsibilities

  • Provide direct nursing care to hospice patients in home and facility settings
  • Assess patient needs and monitor symptoms, vital signs, and changes in condition
  • Administer medications, treatments, and interventions as ordered
  • Educate and support patients and families regarding symptom management and end-of-life care
  • Communicate changes and concerns promptly to the RN and interdisciplinary care team

Requirements

  • Current LPN license valid for practice
  • Current driver's license
  • Minimum of one year of nursing experience
  • Hospice or palliative care experience
  • Reliable transportation and valid automobile liability insurance
