About this role
Registered Nurse Home Health position at Elara Caring. The role involves assessing assigned case load of home health care patients and families, identifying physical, psychosocial, and environmental needs, assuming primary responsibility for caseload including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. The nurse makes initial evaluations and re-evaluates nursing needs during each visit, communicates significant findings to Clinical Supervisor and physician, and revises plans of care as needed.

- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Able and willing to travel within branch/office coverage area
- Dependable vehicle, valid driver's license, and current auto insurance
- Must be able and willing to travel 50%
- Associate's Degree or Bachelor's Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
