Position Overview

We are seeking a compassionate and skilled Nurse Practitioner (NP) with experience in value-based care and managing chronic conditions. The ideal candidate is committed to patient-centered care, works well in a collaborative team environment, and is motivated by improving patient health outcomes while maintaining efficiency in a primary care setting.

Responsibilities

• Provide comprehensive primary care services, including diagnosis, treatment, and management of acute and chronic conditions.
• Focus on preventive health measures and patient education to support long-term wellness.
• Manage a panel of patients under a value-based care model, emphasizing quality metrics and cost-effective care.
• Collaborate with physicians, medical assistants, and care coordinators to develop individualized care plans.
• Conduct annual wellness visits, chronic care follow-ups, and medication management.
• Accurately document encounters in the electronic health record (EHR) in compliance with organizational and regulatory standards.
• Participate in quality improvement initiatives and contribute to clinical protocols that enhance patient outcomes.

Qualifications

• Current and unrestricted Nurse Practitioner license in the state of Florida.
• Board Certification (Family or Adult NP preferred).
• Minimum of 2 years of experience in primary care, with strong background in chronic condition management (e.g., diabetes, hypertension, COPD, CHF).
• Familiarity with value-based care models (Medicare Advantage, ACOs, or risk-based contracts preferred).
• Excellent communication, interpersonal, and teamwork skills.
• Strong organizational and time management abilities.
